Gracious Hearts Home Care is now fully contracted with TriWest and the VA.
Homemaker, Home Health Aide or In home Respite Care is available through TriWest CCN. This care involves help with daily activities such as bathing, grooming, meal preparation, and household tasks to enable independence and safety for the Veterans who are in need of it.
This care service must be approved by VA, who chooses an appropriate SEOC. the SEOC indicates the number of hours the Veteran qualifies for each week. The specific amount of time that is ordered by the VA physician is then sent as a referral to the Home Care of choice or a random assigned home care approved for those specific hours. We can guide each veteran on the steps to get started with this service and how to get the referral to the home care of your choice.
We can assist Veterans and their widows in applying for Aid and Attendance benefits as well. Aid and Attendance is a benefit in which veterans or surviving spouses could receive monthly VACCN benefits.
VACCN- Veteran’s Community Care Network is a benefit that pays for care in the home. There may be asset limitations and 1 year of active duty requirements in order to receive this benefit.
