Veteran's Care Service

Receive up to 20 hours of Homemaker/Respite Care

Gracious Hearts Home Care provides trained caregivers to assist Veteran's in their home settings for Homemaker/Respite Care Services. All enrolled Veterans are eligible for Homemaker Home Health Aide Care if they are eligible for community care and meet the clinical criteria for the service. 

Homemaker/Respite Care Services

How is it determined?

Services are based on your assessed needs. Your VA social worker will determine the approval and specific amount of help you may be able to receive. 

What services can you receive?

Daily activities you may be able to receive include:

  • Feeding

  • Dressing assistance

  • Personal Grooming

  • Bathing

  • Bathroom assistance

  • Grocery Shopping

  • Transfer Assistance

Am I Eligible?

The Veteran must be eligible for Community Care and under the care of Veteran's Health Administration. The Veteran must meet clinical criteria for the service. Service may vary based upon location

How to Decide?

There are tools to assist with the decision of Care at Home or in the community to help you figure out what home care services or long term ca services may best met your needs now or in the future. 

The caregiver of the Veteran may also use https://www.va.gov/Geriatrics/docs/Caregiver_Self_Assessment.pdf to help identify their own needs and decide how much support they can offer to the Veteran.  This information along with the guidance of the care team and social worker, will help reach short-term and long-term decisions. If the Homemaker/ Respite Care Services are right for you, the VA social worker can assist with the arrangements.
